What is Form 80?
Form 80 asks for a detailed personal history: every address you've lived at, every trip you've taken, your full employment history, family details, and any involvement with military, government, or law enforcement organisations. It's used by the Department to assess character and identity as part of the visa decision.
When is it requested?
Form 80 isn't part of every visa application — it's typically requested by a case officer after lodgement, or listed as a required document for certain visa subclasses, including skilled visas (189, 190, 491, 482), partner visas (820/801, 309/100), and some student and family visas. If it's requested, treat it as mandatory: incomplete or inconsistent responses are a common cause of processing delays.
What it covers
- Personal and identity details, including all previous names
- Every residential address for the last 10 years
- Full employment history, including periods of unemployment
- All international travel in the last 10 years
- Family members, including siblings and their whereabouts
- Military, government, and law enforcement service or involvement
- Any criminal history, charges, or convictions, in any country
Answer every question, even if the answer is 'none' or 'not applicable'. Leaving a section blank looks like an oversight to a case officer — an explicit 'N/A' shows you've considered it.
